CONCERNS

Coordination is difficult. If you are caring for elderly parents at a distance, you are not alone. Today, nearly seven million Americans are long-distance caregivers to aging parents and other loved ones.

While this is admirable, the increasing complexity of health and financial issues facing today’s elderly can become overwhelming. The stress of trying to negotiate the maze of financial, social, and health-related issues can often put strain on the parent-child relationship.

Our Senior Care Management Program can provide everyone peace of mind.  Our Care Manager, with expertise in social work, gerontology and counseling, can oversee your loved one’s care, coordinate needs with medical and financial professionals, and continually communicate with family members. Families often think of their Care Manager as a member of the family, which is especially reassuring when their loved one is far away.

  • Cost-control.  The Care Manager strives to reduce inappropriate institutional care and overuse of emergency services. We match the service to the senior’s needs and help prevent crises and unnecessary hospitalizations.
  • Quality control.  We have adopted quality standards of practice, based on The National Academy of Certified Care Managers

SERVICES

  • A comprehensive assessment
    • Medical     – history, present illnesses, surgeries, medications, etc
    • Emotional/Social – cognitive and mental status
    • Functional – ability to perform activities of daily living safely
    • Environmental – current living situation, including nutrition and home safety
    • Legal – status of  documents such as Advanced Directives and Durable Power of Attorney
    • Financial – income/resources to determine eligibility for services and assistance
  • An individualized plan of care – Address areas of concern and outline our recommendations and goals
  • Arrangement of services – Coordinate and monitor home care services, adjusting them according to changes in level of need
  • Accompaniment to medical appointments and emergent care – Inform medical providers, advocate for senior, assist with complexity of medical instructions
  • A family liaison – Act as a supportive link between the senior and family to improve and streamline communication efforts

PAYMENT

While most Senior Care Management services are self pay, some services may be covered by a senior program or a long term care insurance.  If so, we will assist you with both the authorization and the billing process.  Services are billed twice per month.
